
Vitamin D is often discussed in relation to bone health, but it also has plausible links to cardiovascular health. Low Vitamin D May Be a Marker, Not a Cause
One reason the evidence is difficult to interpret is that low vitamin D levels may reflect broader health patterns rather than directly cause heart disease. People with less sun exposure may spend less time outdoors because of poor health, limited mobility, or more sedentary lifestyles. They may also have obesity, chronic illness, or dietary patterns associated with worse cardiovascular outcomes.

What the TARGET-D Study Found
The TARGET-D study added a more targeted approach. It followed 630 people who had recently suffered a heart attack. Participants were assigned either to standard care or to individualized vitamin D dosing designed to raise blood levels into a target range of 40 to 80 ng/mL.
After about four years, the group receiving tailored vitamin D had roughly half the risk of a second heart attack and fewer major cardiovascular events overall. Many participants needed about 5,000 IU daily to reach the target range.
These findings matter for several reasons:
The study focused on a high-risk group
People who have already had a heart attack are at elevated risk for another event. If vitamin D is helpful anywhere, this is the kind of population in which an effect might be most detectable.
The dosing was individualized
Rather than giving everyone the same amount, investigators aimed for a biological target. That design better reflects clinical practice in which treatment is adjusted to lab values.
The outcome was clinically meaningful
A reduction in recurrent heart attacks is not a trivial endpoint. It is one of the most important outcomes in cardiovascular medicine.
Still, the study has important limitations.
Why the New Study Is Encouraging but Not Final
The TARGET-D results are promising, but they should be interpreted carefully.
The sample size was relatively small
A study of 630 people can detect meaningful patterns, but it is not large enough to establish a new standard of care for millions of adults.
The study population was narrow
These were people who had already experienced a heart attack. The results may not apply to people who have never had cardiovascular disease.
More evidence is needed
A single positive study, especially when earlier trials were neutral, is not enough to conclude that vitamin D supplementation prevents heart attacks in the general population.
The target range may matter
The study did not merely ask whether taking vitamin D helps. It asked whether raising blood levels into a specific range helps. That distinction is important. It suggests that blood level, baseline deficiency, and dose may all affect outcomes.

Who Might Benefit Most
If vitamin D is going to matter for cardiovascular health, it is most likely to matter in people with:
- Confirmed vitamin D deficiency
- Prior myocardial infarction, also called heart attack
- Limited sun exposure
- Malabsorption or other conditions that affect nutrient absorption
- Obesity, which can alter vitamin D distribution
- Older age or other risk factors for deficiency
That does not mean everyone in these groups should take high-dose vitamin D automatically. It means they are the people in whom testing and individualized care are most reasonable.
Why You Should Not Start High-Dose Vitamin D on Your Own
Vitamin D is fat-soluble, so the body stores it rather than excreting excess readily. Too much can lead to toxicity. The main concern is elevated blood calcium, or hypercalcemia, which can cause:
- Nausea
- Constipation
- Weakness
- Confusion
- Excessive thirst
- Frequent urination
Over time, excessive vitamin D can also contribute to kidney stones and kidney injury.
This is why the apparent promise of TARGET-D should not be interpreted as permission to self-prescribe 5,000 IU or more per day. The right dose depends on current blood levels, age, medical history, body size, and concurrent medications.
A Practical Way to Think About Vitamin D and Heart Attack Risk
A balanced interpretation is often the most useful one.
Vitamin D is not a substitute for standard cardiovascular prevention
People at risk for heart attack still need evidence-based care, including:
- Blood pressure control
- Cholesterol management
- Diabetes management
- Smoking cessation
- Exercise
- Weight management
- Appropriate antiplatelet or statin therapy when indicated
Vitamin D should be viewed, at most, as a possible adjunct in selected patients, not a replacement for established prevention.
Testing matters more than guessing
If there is concern about deficiency, a blood test can clarify whether a level is low. That is preferable to taking a large dose without knowing the baseline value.
More is not always better
A higher number on a supplement label does not automatically mean a better outcome. The goal is to correct deficiency safely, not to maximize intake.
What This Means for People Who Have Had a Heart Attack
For someone recovering from a myocardial infarction, the key question is not whether vitamin D is “good” in the abstract. The question is whether correcting deficiency, if present, could reduce the chance of another event.
The TARGET-D findings suggest that for some patients, especially those with low levels, individualized supplementation may help. But because the evidence is still emerging, this should be handled as part of a broader post-heart-attack care plan, not as a stand-alone strategy.
A reasonable conversation with a clinician might include:
- Whether a vitamin D test has been done recently
- Whether the result suggests deficiency or insufficiency
- Whether supplementation is appropriate
- How often levels should be rechecked
- Whether the dose should be adjusted over time
